Patient Safety
Organization The MHA Patient Safety Organization (PSO) seeks to improve patient safety
and quality for all Michigan residents. The MHA PSO is listed by
the Agency for Healthcare Research and Quality as a certified PSO and is
a component of the MHA Health Foundation.
In addition to
Michigan-specific activities and partners, the MHA PSO works with ECRI
Institute PSO to provide its members with resources and experiences that
span the United States.
The MHA PSO has, over the past several
years, worked to develop data collection and analysis tools, educational
opportunities, safety improvement activities and more to identify areas
of health care in need of improvement, share experiences and implement
change to prevent future patient harm.
The MHA PSO, while
constantly collecting serious reportable events from hospitals that
voluntarily submit them, is also implementing safety improvements
including, but not limited to:

Safe Tables: A "Safe Table" is a
members-only, shared learning meeting among health system and hospital
peers to exchange patient safety experiences, discuss best practices,
network and learn in an open, uninhibited and legally protected
environment. The venue provides attendees with the opportunity to share deidentified adverse events with other hospitals, gain input and
support, and learn from their own mistakes as well as the mistakes of
others.

Engaging providers from across the continuum:
recognizing the strong connection between health outcomes and care
transitions, the MHA PSO has expanded its membership to include
long-term care providers as well as organ donation experts.

Simplifying the data submission process: the MHA PSO is working with member
hospitals to implement automated data transfer to make the serious
reportable event submission process faster, easier and more efficient.

Supporting the work of the MHA Keystone Hospital Engagement
Network (HEN): specifically, the MHA PSO is managing three of the 10
areas of focus of the HEN: adverse drug events, falls and pressure
ulcers.
MHA PSO-member providers that wish to submit a serious
reportable event or access other members-only tools should log in to
the
secure member portal.
